Wrinkles

Wrinkles develop through a gradual loss of collagen, elastin and hyaluronic acid, combined with changes in the architecture of the skin. As skin ages, fibroblast activity declines, cell renewal slows and the skin's natural capacity to repair itself diminishes. Intrinsic ageing, UV exposure, lifestyle, facial expression and genetic predisposition all contribute to the breakdown of connective tissue.

The result is thinner, less elastic skin in which fine lines gradually deepen into static wrinkles. Depending on the cause, the depth of the wrinkles and the degree of skin laxity, every client calls for a different treatment strategy. It is not only the severity of the ageing that determines which technology is most appropriate, but also the acceptable recovery time, the skin type and the client's expectations.

Technologies for wrinkle treatment

Modern energy-based technologies share a single objective: stimulating new collagen and elastin and improving the architecture of the skin. How they achieve it, however, differs considerably from one technology to the next.

Non-ablative fractional lasers such as the MOSAIC 3D create controlled thermal microzones deep in the dermis without removing the epidermis. This activates the natural wound-healing response and produces gradual collagen remodelling with minimal downtime. The treatment is particularly suited to early signs of ageing, fine lines, and clients who want to maintain their skin quality preventively.

Sub-ablative lasers such as the LaseMD Ultra sit between non-ablative and fully ablative resurfacing. By combining controlled microscopic channels with thermal stimulation, they provoke a powerful collagen response while keeping recovery considerably shorter than with traditional ablative treatments. This makes sub-ablative treatments particularly suitable for moderate wrinkling and for clients who want a clearly visible result with relatively limited downtime.

For more pronounced ageing, laxity and deeper static wrinkles, ablative fractional lasers such as the eCO2 3D and the UltraClear offer the most intensive form of skin renewal. Removing microscopic columns of tissue in a controlled way triggers a powerful regenerative response in which both the epidermis and the dermis are renewed. The result is a substantial improvement in wrinkle depth, skin texture and skin quality, with a longer recovery period as the logical trade-off.

RF microneedling, as offered by the Genius®, combines mechanical microneedles with radiofrequency energy delivered deep into the dermis. This stimulates collagen and elastin production without treating the entire skin surface. The technique is particularly effective for fine to moderately deep wrinkles, skin firming, and areas where additional lift is wanted, such as the neck, jawline and décolletage.

Dual radiofrequency represents a new generation of skin rejuvenation in which two radiofrequencies treat different skin layers simultaneously. Stimulating superficial and deeper structures at the same time produces powerful remodelling of the collagen network. Alongside improving fine and deeper wrinkles, this technology contributes to skin firming, contour improvement and a visible lifting effect — without damaging the epidermis and with minimal recovery time.

The value of the right treatment strategy

No single technology is the best choice in every situation. Younger patients with early signs of ageing are often served well by non-ablative treatments, while patients with deeper static wrinkles or pronounced photoageing benefit more from sub-ablative or ablative resurfacing. Where skin laxity plays an important role alongside wrinkles, RF microneedling or dual radiofrequency can be a valuable addition — or even the primary treatment.

Different technologies are increasingly combined as a deliberate strategy. Not because combining automatically produces better results, but because each technique works through a different biological mechanism. Matching the treatment to the indication, the condition of the skin and the patient's objective produces a personalised treatment plan that maximises both effectiveness and safety.
